HOW DO I....

If you are requesting authorised repeat prescriptions, please allow two working days' notice before collecting your prescription from the surgery or 72 hours if collecting your medical from a local pharmacy. This will give the doctor time to complete the necessary review of your medication before signing the prescription. This service is available Monday to Friday (excluding bank holidays).

A computerised prescription request slip should be brought to the surgery each time a further prescription is required. A box for your completed slip is provided at reception. You can also fax (01482) 649513 your request or post it. If you would like it posted back to you please enclose a stamped addressed envelope but do make allowance for possible postal delay.

It is also possible to order repeats online via but you will first need to register for this at reception where you will be issued a user name and password.

Telephone requests for repeat prescriptions are reserved for elderly and housebound patients only.


We are now able to send your prescription electronically to any participating pharmacy of your choice.

If you are happy for your prescription to be sent electronically, please complete the relevant form (which is available from reception). You can also sign up to the electronic prescription service at any participating pharmacy.

If you change your mind, at any time, about using this service there is no problem at all. You can cancel or change your chosen pharmacy at any time by just letting the pharmacy or the practice know.

Please be aware there are some prescription items that cannot be sent electronically. We will continue to produce these in paper format and forward to your chosen pharmacy.

HOW DO I....

We only accept new patients who reside within our catchment area. The practice catchment area is that west of the Hull which includes the villages and townships of Hessle, Anlaby, Anlaby Common, Kirk Ella, Swanland, North Ferriby, Welton and Melton.

To register as a patient please complete the forms available at the reception desk – each patient must complete an individual form.

Patients have a right to express a preference of practitioner when submitting their registration documents. Whilst the practice will endeavour to comply it may not always be possible and if that were to be the case an explanation would be offered.

The following forms are included in your registration pack and all sections must be completed in full (if any entries are missing we will be unable to process your application):

  • Family doctor registration form (GMS1)
  • New patient registration form
  • Summary Care Record (information and opt out form)
  • Text messaging service consent/opt out form

To enable registration to proceed you will be required to provide proof of identity and proof of address with your registration application.


The practice generally cannot accept registrations from patients who live outside the catchment area.

The practice cannot accept registrations from patients if the list is currently ‘closed’.

Under 18s must register with a parent or guardian.

The practice may on other rare occasions refuse to accept registrations but will not do so on grounds that relate to the applicant’s race, gender, social class, age, religion, sexual orientation, appearance, disability or medical condition.

Patients who have been removed from the practice list as a result of abuse or aggressive behaviour or have left because of a breakdown in the GP/patient relationship, will not be re-registered.

Patients who are refused registration should contact NHS East Riding of Yorkshire CCG's patient registration section on (01482) 650700 for help with finding an alternative practice or can visit for general information about GP surgeries in their residential area.
