OBTAIN A REPEAT PRESCRIPTION?

If you are requesting authorised repeat prescriptions, please give two working days' notice. This service is available Monday to Friday.

A computerised prescription request slip should be brought to the surgery each time a further prescription is required. A box for your completed slip is provided at reception. You can also fax (01482) 649513 your request or post it. If you would like it posted back to you please enclose a stamped addressed envelope but do make allowance for possible postal delay.

It is also possible to order repeats online via https://systmonline.tpp-uk.com but you will first need to register for this at reception where you will be issued a user name and password.

Telephone requests for repeat prescriptions are reserved for housebound patients only.

REGISTER WITH THE PRACTICE?

We only accept new patients who reside within our catchment area. The practice catchment area is that west of the Hull which includes the villages and townships of Hessle, Anlaby, Anlaby Common, Kirk Ella, Swanland, North Ferriby, Welton and Melton.

To register as a patient please complete the forms available at the reception desk – each patient must complete an individual form.

The following forms are included in your registration pack and all sections must be completed in full (if any entries are missing we will be unable to process your application):

Family doctor registration form (GMS1)
New patient registration form
Summary Care Record (information and opt out form)
Text messaging service consent/opt out form

To enable registration to proceed you will be required to provide proof of identity and proof of address with your registration application.

REGISTRATION REFUSALS

The practice generally cannot accept registrations from patients who live outside the catchment area and for clinical management reasons we generally do not accept registration of only part of a family group who are all living at the same address.

The practice cannot accept registrations from patients if the list is currently ‘closed’.

Under 18s must register with a parent or guardian.

The practice may on other rare occasions refuse to accept registrations but will not do so on grounds that relate to the applicant’s race, gender, social class, age, religion, sexual orientation, appearance, disability or medical condition.

Patients who have been removed from the practice list as a result of abuse or aggressive behaviour or have left because of a breakdown in the GP/patient relationship, will not be re-registered.
